- How much longer will central-line therapy continue? Catheter repair may be more appropriate if therapy will continue for a few days, whereas replacement is more appropriate for longer-term therapy.
- Are vessels available for a new insertion? Did the inserting clinician note that the catheter was inserted with much difficulty? Does the patient have a history of multiple catheter insertions?
- Is the catheter appropriate for exchange? Is there a chance of contamination or infection with this catheter or insertion site?
- Is repair feasible based on variables of catheter damage and exposure? What are the possible risks of contamination and infection?
- What are the manufacturer’s recommendations? Many catheters don’t come with repair kits. Depending on leakage or breakage location, repair may be impossible—for instance, with a double-lumen catheter that’s leaking below the bifurcation.
- If the catheter is visibly ruptured on the outside, is there a possibility it has ruptured on the inside? This can be determined only by X-ray or dye study.

Voiding cystourethrogram (VCUG) in neonates with first-time febrile UTI (September 2017)
For neonates with first-time urinary tract infection (UTI), the optimal approach to evaluating for vesicoureteral reflux (VUR) is uncertain, and expert opinion differs as to whether a voiding cystourethrogram (VCUG) is necessary in all cases. A recent study of 122 young infants with first-time febrile UTI found that high-grade VUR was far less likely among infants with normal renal ultrasound and Escherichia coli infection compared with those with abnormal renal ultrasound, non-E. coli pathogen, or both abnormal ultrasound and non-E. coli pathogen (1 versus 31, 26, and 55 percent, respectively) [18]. These data suggest that a VCUG may not be necessary for all neonates with first-time UTI, and they support a strategy of "watchful waiting" for low-risk infants. However, VCUG should be performed in neonates with abnormal renal ultrasound, non-E. coli pathogen, or recurrent UTI

Peak respiratory rate (RRpeak) at the first 36 h was significantly higher in group 2 (P > 0.000). The cut-off for RRpeak during the first 36 h (RRpeak36) was 90/min and RRpeak36 > 90/min caused a 7.04-fold risk of prolongedtachypnea. White blood cell count and hematocrit levels were lower whereas duration of hospitalization and antibiotic treatment were longer in group 2.
Assessment of RRpeak36 may be useful in predicting clinical course of TTN.
Initially prolonged RVSTI RT VENTR SYTOLIC TIME INTERVAL was best predictor of the development of severe TTN (relative risk ratio 17.5, p less than 0.001): clinical characteristics and oxygen requirements at the admission had limited predictive value.
