There also seems to be a set point for morality—a point at which we feel that we’re being “good enough” and then stop trying. One study, for example, had a group of people imagine performing some good deed (cleaning a park area of a university). The people who did the imagining were then more likely to cheat on a game that would give them money than a control group, who did not imagine performing a good deed beforehand. This suggests that even thinking about doing something good can get you to let yourself morally off the hook, free to do something bad. What’s more, people who buy “green” products are, everything else being equal, subsequently more likely to cheat and steal. This offsetting of good behavior is known as moral licensing.

SET POINTS -HOMEOSTASIS
Set points appear at many levels, and our survival usually depends on them not changing too drastically. Our body, for example, has many regulatory systems that work like a thermostat, keeping our bodies in equilibrium. Your brain has to stay near 98.6 degrees Fahrenheit or really, really bad things will start to happen. If it rises only a few degrees, you start convulsing, and a few degrees more and you’re dead. Your body has several strategies for maintaining the optimum temperature: increasing or decreasing blood flow through dilation, decreasing brain activity, or even the outright destruction of nerve cells. We have similar systems at work for pH levels, our endocrine system, and many others.

When you practise in this way all thoughts of sleeping, eating and even dying will be forgotten. One just dwells single-pointedly on the hua-t’ou to which one has to awaken. If you penetrate the hua-t’ou with great determination, the path of words is cut off and any place for the mind to dwell disappears. It now becomes very transparent, bright and clear. At this time the mature person no longer has any doubts about the words of the great masters. But upon awakening in such a way, the time has come when he must go and see a realized teacher to determine whether his awakening is genuine or not. This is the moment when one has to cut and polish one’s understanding with a good teacher. Yet to people who do not know, one does not say a word. One behaves like a dumb and stupid person. Upon meeting someone else who also knows, a simple movement of the eyes is enough for each to recognize the level of the other’s understanding. In these ways such people respect one another.

Now that I’ve practiced for a decade primarily seeing newborns and children, I’ve had many conversations with parents in which I have to convey the news that their child is dying. If there is one fear common among many of these parents, it is that their child will not have lived long enough to have made an impression on anyone outside the family. This is a particular fear among NICU parents of moribund newborns. I try to always call the baby by their name with the intent of indicating to their parents that their newborn’s existence matters, no matter how brief that existence may be.

The Oxford English Dictionary defines depersonalization as “the action of divesting someone or something of human characteristics or individuality.” It is used to train soldiers and punish prisoners, in both cases with the goal of breaking down the person by distorting their sense of self.

DEPRESSION-SIGECAPS
- Sleep? I definitely oscillated between insomnia and hypersomnia.
- Interest? Patients, doctors, the hospital were all becoming commonplace. Throughout medical school, I’d jotted down stories of patients and lessons I’d learned. I thought about the last few months and realized entries had ceased (not because I was busy, but because I was no longer interested in what patients could teach me.)
- Guilt? I was without a role or purpose. I spent months “pretending to be a doctor” without real input on patient care but with constant pressure to perform and make a positive impression. The reality was my presence felt meaningless to patients and staff alike.
- Energy? Gone. Workouts had fallen by the wayside sometime throughout the year. Making it through a shift was all I could ask of myself.
- Concentration? I had always enjoyed studying and research, but it seemed I could not focus on topics that interested me (i.e., anemia) let alone those that didn’t (i.e., electrolyte disorders).
- Appetite? The question which triggered this exercise in self-diagnosis, most definitely gone.
- Psychomotor symptoms? I don’t believe I was manifesting this symptom — of course, I’d always had trouble comprehending exactly what qualifies as psychomotor.
- Suicidal? No, but I wonder, if I was, would people notice?
