Vulvar involvement is a rare manifestation of extraintestinal CD and exceptionally rare as the first manifestation in children.1 One of the main clinical features is an asymmetrical labial swelling, frequently confused with infectious or traumatic vulvitis.2 ,3 Non-caseating granulomatous inflammation in superficial papillary and deep reticular dermis on biopsy confirms the diagnosis.2 ,3 Given the rarity, treatment targets the underlying disease process, and previous reported cases have a short follow-up, making it difficult to predict long-term prognosis
