BPLS 100 scenarios

 Below are 100 simulation scenarios suitable for APLS (Advanced Paediatric Life Support) or general resuscitation training.

They are educational, descriptive scenario prompts — not real-world treatment instructions.

You can use them for drills, assessments, moulage design, communication training, or teamwork practice.


I’ve divided them into Cardiac, Respiratory, Neurological, Trauma, Toxicology, Infectious, Metabolic, and Special Situations for easy use.





🔴 

CARDIAC – 25 Scenarios



  1. Infant with sudden cyanosis and poor perfusion suspected duct-dependent congenital heart disease.
  2. Child collapses during sports with exertional syncope and suspected hypertrophic cardiomyopathy.
  3. Toddler with fever and tachycardia progressing to compensated shock.
  4. Neonate with bradycardia after hypothermia exposure.
  5. 6-year-old with palpitations and narrow-complex tachycardia.
  6. 12-year-old with chest pain after viral prodrome suspected myocarditis.
  7. Infant with wide-complex tachycardia and poor pulses.
  8. Postoperative congenital heart patient with sudden desaturation and hypotension.
  9. Child with suspected Kawasaki disease presenting with shock.
  10. 8-year-old with syncope while swimming, concern for long QT syndrome.
  11. Toddler with severe anaemia causing tachycardia and collapse.
  12. Infant with persistent crying leading to breath-holding spell and transient cyanosis.
  13. Neonate with poor feeding, sweating, tachypnea and suspected heart failure.
  14. Ventricular fibrillation after lightning strike.
  15. Chest trauma in adolescent leading to commotio cordis.
  16. Adolescence with overdose of a cardioactive medication causing arrhythmia.
  17. Infant with SVT unresponsive to initial maneuvers.
  18. Child with hypotension after severe dehydration secondary to gastroenteritis.
  19. Post-viral pericarditis with muffled heart sounds and deteriorating perfusion.
  20. Syncope in school-aged child triggered by emotional stress — suspected arrhythmogenic cause.
  21. Newborn with cyanotic spell and increased work of breathing.
  22. Child with repaired Tetralogy presenting with acute cyanosis.
  23. Pre-teen with acute chest pain after epinephrine autoinjector misuse.
  24. Child with fever and rash progressing to toxic shock physiology.
  25. Infant with possible anomalous coronary artery causing sudden collapse.

RESPIRATORY – 15 Scenarios



  1. Severe asthma attack with exhaustion signs.
  2. Infant with bronchiolitis and impending respiratory failure.
  3. Anaphylaxis with rapidly progressing airway swelling.
  4. Child with foreign body airway obstruction gradually progressing to cyanosis.
  5. Teenager with pneumonia and sepsis physiology.
  6. Toddler with severe croup and stridor at rest.
  7. Near-drowning child with worsening respiratory distress.
  8. Child with smoke inhalation from housefire.
  9. Neonate with congenital diaphragmatic hernia presenting decompensation.
  10. Asthmatic child deteriorating post-viral infection.
  11. Child with allergic reaction to peanuts causing wheeze and hypotension.
  12. Infant with apnea episodes after RSV infection.
  13. Teen with spontaneous pneumothorax and chest pain.
  14. Child with neuromuscular disorder and acute respiratory fatigue.
  15. Toddler with chemical inhalation from household cleaning fluids.

NEUROLOGICAL – 10 Scenarios



  1. Febrile seizure with prolonged post-ictal state.
  2. Child with head injury followed by decreasing consciousness.
  3. Toddler with sudden ataxia and lethargy suspected ingestion.
  4. Adolescent with acute severe headache and photophobia.
  5. Infant with bulging fontanelle and irritability suggesting raised ICP.
  6. Seizure due to electrolyte imbalance.
  7. Child with status epilepticus after missed medications.
  8. Teen with syncope and transient confusion after sporting event.
  9. Child with suspected meningitis and early septic shock signs.
  10. Toddler with suspected non-accidental trauma presenting with altered mental status.






🟢 

TRAUMA – 25 Scenarios



  1. Bicycle accident with open femur fracture and hypotension.
  2. Child struck by falling object with scalp laceration and vomiting.
  3. Motor vehicle collision — child in car seat with abdominal bruising.
  4. Fall from height with bilateral leg deformities and tachycardia.
  5. Penetrating chest injury with unilateral breath sounds.
  6. Football tackle leading to suspected cervical spine injury.
  7. Toddler with burns over multiple body regions.
  8. ATV rollover with suspected pelvic fracture.
  9. Skateboarding fall causing wrist deformity and pallor.
  10. Teen assault victim with multiple contusions and confusion.
  11. Playground fall leading to limb fracture and dizziness.
  12. Pedestrian hit by car with altered consciousness.
  13. Accidental hanging from rope swing with airway concern.
  14. Bicycle handlebar injury causing abdominal pain and hypotension.
  15. Firework blast injury with facial burns.
  16. Dog bite to the face in a preschooler.
  17. Severe nosebleed after facial trauma with airway compromise risk.
  18. Fall into empty pool with spinal pain and leg weakness.
  19. Adolescent struck in chest by baseball with collapse.
  20. Tree-branch puncture wound to thigh with significant bleeding.
  21. Child ejected from go-kart with suspected chest injury.
  22. Sledding accident causing blunt abdominal trauma.
  23. Heavy object crush injury to lower limbs.
  24. Playground entrapment causing limb ischemia.
  25. Sports concussion with progressively worsening symptoms.






🧪 

TOXICOLOGY – 10 Scenarios



  1. Medication ingestion (unknown pills) with drowsiness and hypotonia.
  2. Iron tablet overdose in toddler.
  3. Household cleaner ingestion with drooling and airway concern.
  4. Teenager with intentional overdose of common analgesics.
  5. Carbon monoxide exposure in a family, child most affected.
  6. Toddler with cannabis ingestion, lethargic and unresponsive.
  7. Pesticide exposure causing secretions and bradycardia.
  8. Teen vaping unknown substance leading to respiratory distress.
  9. Child bitten by snake with swelling and shock.
  10. Battery ingestion presenting with drooling and vomiting.






🧬 

METABOLIC / ENDOCRINE – 8 Scenarios



  1. Severe dehydration from gastroenteritis leading to shock.
  2. Diabetic ketoacidosis in teen with abdominal pain and confusion.
  3. Hypoglycemia in infant with seizures.
  4. Newborn with suspected inborn error of metabolism presenting lethargy.
  5. Electrolyte imbalance causing arrhythmia symptoms.
  6. Addisonian crisis with low blood pressure and vomiting.
  7. Hypernatremia from improper formula mixing.
  8. Refeeding syndrome signs in malnourished adolescent.






🦠 

INFECTIOUS / SEPSIS – 7 Scenarios



  1. Child with high fever, petechiae, and rapidly dropping blood pressure.
  2. Neonatal sepsis with temperature instability and apnea.
  3. Child with osteomyelitis presenting with worsening systemic signs.
  4. Severe dehydration and shock from cholera-like illness.
  5. Child with malaria returning from travel with altered mental status.
  6. Post-surgery wound infection progressing to sepsis.
  7. Immunocompromised teen with neutropenic fever and rapid decline.
