- Under 3 months old: add amoxicillin to cover Listeria.
- •Use cefotaxime (not ceftriaxone) in premature or jaundiced infants, in hypoalbuminaemia and if calcium infusion is being used.
- •If hospital acquired or neutropenic, consider tazobactam.
- •If previous culture of resistant organism, give appropriate antibiotic (e.g. methicillin-resistant Staphylococcus aureus (MRSA): add vancomycin; extended-spectrum β-lactamases (ESBL): meropenem).
- •If vascular access device has been used for more than 48 hours add vancomycin.
