LONDON, Oct 9 (Reuters) - Quick treatment of minor strokes can greatly reduce the chance of a major stroke later, but most patients are not getting care in time, according to two studies published on Tuesday.
The researchers showed that immediately treating small strokes cut the risk of a disabling or deadly stroke in the next 30 days by about 80 percent, said Peter Rothwell, a neurologist at the University of Oxford, who led one of the studies.
Most follow-on strokes happen within 30 days of a smaller one, but most patients do not get treatment for weeks, he said.
"These mini-strokes are essentially warning events," he said in a telephone interview.
Strokes, which occur when blood flow to the brain is blocked, can kill brain tissue and are one of the worldwide leading causes of death and permanent disability. Treatments include blood thinning drugs and attempts to lower cholesterol.
The researchers were looking at so-called ischaemic strokes, caused by a blood clot, and not the more unusual haemorrhagic strokes, caused by bleeding in the brain.
In the study, published in The Lancet medical journal, researchers found the risk of a major stroke within the first month following a minor one fell to about 2 percent from 10 percent for patients who received treatment almost immediately.

BABY POWDER

A group of friends on vacation go for a day hike in the Alps. Half way to the mountain top, which is their goal, they turn a bend in the path and find their way blocked by a huge boulder that has fallen in such a a manner that it cannot be dislodged and cannot be circumvented. The first hiker’s stomach sinks in disappointment. “That’s it,” he says, ” The hike’s over!” …. From Sartre’s point of view, this person has chosen the facticity of the boulder as an insurmountable obstacle and chosen himself as defeated. A second hiker begins photographing the rock, excited by its sublime power and by the beauty of the landscape framing it. She has chosen the boulder as aesthetic object and chosen herself as a recorder of beauty - that is, as an artist. A third hiker examines the boulder scientifically, noting its mineral composition and the impact of its recent fall on the path. For her, this boulder is a motive for scientific study and is the occasion for her to act as a scientist. The fourth hiker says, “There is got to be a way around this thing,” and begins an series of experiments to overcome the obstacle. He has chosen the boulder’s facticity as a challenge and himself as hero.
The determinist argues that there must be something in the past of each of these hikers that determined their response. Sartre denies this. There is nothing in the facticity of the past of any of the hikers, nor in the facticity of the boulder, that necessitates any particular response to the boulder’s presence. For Sartre, the facticity of the rock is undeniable, but each person chooses the MEANING of that facticity for him or herself. Because facticity in itself is meaningless, the source of the meaning is a decision on the part of the individual. There are always alternative interpretations for meaning available; we are never confronted with only one possible choice.
There is always the most radical choice of all - the choice of death. A hiker might decide that the boulder’s presence is so depressing that he cannot go an living. This would of course be an ABSURD response to the boulder’s facticiy, but its mere possibility shows Sartre that all other responses were chosen as alternatives to death. If you did not kill yourself this morning (and apparently you didn’t) then you choose an alternative to death…. and you are responsible for that choice and for its consequences.

Patricia DONOVAN-DUFF, RN: I think she would call them phases, not stages or steps, because it's not a linear process in grieving. It's definitely a rollercoaster kind of experience. It's not that you finish with one phase and move on to the next one. You can retreat back and forth. She did identify kind of similar phases but, again, they're not linear.

The decision to treat is generally based on the triglyceride levels themselves. Normal triglyceride levels are less than 150 mg/dL. Borderline high levels are 150-199 mg/dl. High levels are 200 - 499 mg/dL, and very high triglyceride levels are greater than 500 mg/dL. For people with borderline or high triglyceride levels, treatment should emphasize weight reduction and exercise. Drugs are recommended for people with very high triglyceride levels. Most people who need treatment for high triglyceride levels have metabolic syndrome X.




///////////////////////Statins: Several statin drugs are now on the market, including lovastatin, pravastatin, atorvastatin and simvastatin. These drugs inhibit the liver enzyme HMG-CoA reductase, which significantly reduces the production of cholesterol by the liver. These drugs result in a significant reduction in LDL cholesterol, with a modest decrease in triglycerides, and a modest increase in HDL cholesterol. They tend to be well-tolerated in general, but can cause elevations in liver enzymes (which therefore need to be monitored). They can also cause a muscle disorder which can be severe in rare individuals. The muscle disorder (myopathy) is particularly likely when statins are used in combination with gemfibrozil. Of these drug choices, the statins are not only more effective than other categories, they also tend to be much better tolerated. Furthermore, evidence is accumulating that the aggressive use of statins can actually arrest the progression of coronary artery disease, and in some circumstances can be used instead of more invasive procedures such as angioplasty.
The treatment of abnormal lipid levels can be summarized as follows: First, dietary changes, weight loss, and exercise should be tried. If that fails to restore adequate lipid levels, then most doctors will try statins. If statins fail, or if they are not tolerated, an agent from another class of the lipid-lowering drugs can be tried.

Emotions are thought to be related to activity in brain areas that direct our attention, motivate our behavior, and determine the significance of what is going on around us. Pioneering work by Broca (1878), Papez (1937), and MacLean (1952) suggested that emotion is related to a group of structures in the center of the brain called the limbic system, which includes the hypothalamus, cingulate cortex, hippocampi, and other structures. More recent research has shown that some of these limbic structures are not as directly related to emotion as others are, while some non-limbic structures have been found to be of greater emotional relevance. The following brain structures are currently thought to be most involved in emotion:
Amygdala — The amygdalae are two small, round structures located anterior to the hippocampi near the temporal poles. The amygdalae are involved in detecting and learning what parts of our surroundings are important and have emotional significance. They are critical for the production of emotion, and may be particularly so for negative emotions, especially fear.
Prefrontal cortex — The term prefrontal cortex refers to the very front of the brain, behind the forehead and above the eyes. It appears to play a critical role in the regulation of emotion and behavior by anticipating the consequences of our actions. The prefrontal cortex may play an important role in delayed gratification by maintaining emotions over time and organizing behavior toward specific goals.
Anterior cingulate — The anterior cingulate cortex (ACC) is located in the middle of the brain, just behind the prefrontal cortex. The ACC is thought to play a central role in attention, and may be particularly important with regard to conscious, subjective emotional awareness. This region of the brain may also play an important role in the initiation of motivated behavior.
Ventral striatum — The ventral striatum is a group of subcortical structures thought to play an important role in emotion and behavior. One part of the ventral striatum called the nucleus accumbens is thought to be involved in the experience of goal-directed positive emotion. Individuals with addictions experience increased activity in this area when they encounter the object of their addiction.
Insula — The insular cortex is thought to play a critical role in the bodily experience of emotion, as it is connected to other brain structures that regulate the body’s autonomic functions (heart rate, breathing, digestion, etc.). This region also processes taste information and is thought to play an important role in experiencing the emotion of disgust.

The neurotransmitter glutamate revs up the brain, causing neural transmission, whereas the neurotransmitter γ-aminobutyric acid suppresses neural activity. If you have middle-class skills, you're paid by the month or year- e.g., $39,000/year to teach high school in San Francisco. But if you're lucky enough to be born into the upper class, you'll be "paid" once in your life, namely when you inherit a trust fund or estate of some millions of dollars of stocks, bonds, real estate, and other assets.
Parallel to the time frames by which one is "paid" are the time frames with which life is viewed. For a truly poor person, life is coped with day by day. A middle-class person has the means to think further ahead: to next summer's vacation, or a down payment for a house. But a truly rich person, being freed from day-to-day or even year-to-year financial anxieties, can plan a whole lifetime with the assurance of prosperity; the goal is to assure similar prosperity for grandchildren and great-grandchildren. Thus, the poor live and plan by the day, and the rich, by the lifetime.

Dark energy was only observationally confirmed in 1999 and it was a huge surprise for the inflationary picture. There is no rhyme or reason for its existence in that picture: dark energy plays no role in the early universe, according to inflationary theory. Whereas in the cyclic model, dark energy is vital, because it is the decay of dark energy which leads to the next Big Bang.
This picture of cyclic brane collisions actually resolves one of the longest-standing puzzles in cyclic models. The idea of a cyclic model isn't new: Friedmann and others pictured a cyclic model back in the 1930's. They envisaged a finite universe which collapsed and bounced over and over again. But Richard Tolman soon pointed out that, actually, it wouldn't remove the problem of having to have a beginning. The reason those cyclic models didn't work is that every bounce makes more radiation and that means the universe has more stuff in it. According to Einstein's equations, this makes the universe bigger after each bounce, so that every cycle lasts longer than the one before it. But, tracing back to the past, the duration of each bounce gets shorter and shorter and the duration of the cycles shrinks to zero, meaning that the universe still had to begin a finite time ago. An eternal cyclic model was impossible, in the old framework. What is new about our model is that by employing dark energy and by having an infinite universe, which dilutes away the radiation and matter after every bang, you actually can have an eternal cyclic universe, which could last forever.

The theory of risk homeostasis states that an individual has an inbuilt target level of acceptable risk which does not change. This level varies between individuals. When the level of acceptable risk in one part of the individual's life changes; there will be a corresponding rise/drop in acceptable risk elsewhere. The same, argues Wilde, is true of larger human systems (e.g. a population of drivers).

Christopher discovers the dead body of Wellington, his neighbour's poodle, speared by a garden fork. Having been blamed for it, he decides to investigate to clear his name. However, he is severely limited by his fears and difficulties when interpreting the world around him. Throughout his adventures, Christopher records his experiences in a book, entitled The Curious Incident of the Dog in the Night-time. During his investigation, Christopher meets people whom he has never before encountered, even though they live on the same street.
Ed, his father, discovers the book and confiscates it from Christopher, after a brief fight between them. In his search for the hidden book, Christopher uncovers a trove of letters to himself from his mother, dated after she allegedly died — which his father had also concealed. Christopher had been told by Ed that she died of a heart attack, but judging from the letters, Christopher concludes that she is still alive and that his father has lied to him. He is so thoroughly shocked by this fact that he is unable to move, curls up on the bed, vomits, and groans for several hours until his father returns home.
Ed realises that Christopher has read the letters and cleans him up. He then confesses that he had indeed lied about Judy's death and also that it was he who killed Wellington, stating that it was a mistake due to his anger after a heated argument with Mrs Shears.
Christopher, having lost all trust in his father and fearing that he may also try to kill him since he had already killed Wellington, decides to escape from home and live with his mother. Guided by his mother's address from the letters, he embarks on an adventurous trip to London, where his mother lives with Mr Shears.
After a long and confusing journey, evading policemen who have been dispatched to find him (due to Ed, who called the police about his disappearance), and feeling ill due to the overwhelming information from the crowds and the signs in the trains, he finally finds his way to his mother and Mr Shears' home, and waits outside until they arrive.
His mother is happy at his appearance and tries to keep him with her, despite the objections of both Mr Shears and Ed, the latter having tracked Christopher down with the aid of the police. His mother eventually leaves Mr Shears, their relationship apparently broken down because of the conflict over Christopher.
She then moves into a rented room in Swindon and, after an argument with Ed, agrees to let Ed meet with Christopher daily for a little while. However, at this stage, Christopher remains terrified of his father; he hopes Ed will be imprisoned for killing Wellington. The story ends with Ed getting Christopher a pet dog, because Toby,Christopher's pet rat, had died, and promising that he will rebuild trust with Christopher slowly, "no matter how long it takes," in his daily, brief sessions, and Christopher asserting that he will take further A-level exams and attend university. He completes his first mathematics A-level with top grades, with his ultimate goal to become a scientist.

Oct. 1, 2007 -- Got high blood pressure? If you're overweight, modest weight loss might bring your blood pressure down to normal.
Italian researchers reported that news in Tucson at the American Heart Association's 61st Annual Fall Conference of the Council for High Blood Pressure Research.
The University of Pavia's Roberto Fogari, MD, and colleagues asked 220 overweight (but not obese) men and women with stage 1 hypertension to lose at least 5% of their body weight in 6 months.

I've been doing a position I don't care for all week...but itdoes involve repetitive tasks and lots of walking...so, when possible,I let it become a meditation. yesterday it came to me...aches, painsand all...yes, this is not the most preferable way to spend my time,but considering the alternative. .being laid off...it's a plum!And I learn a great deal about myself, and my body. Just spending afew minutes sweeping was a welcome break. Just to do somethingdifferent for 5 minutes!I remind myself that even though I am inside, I am still part of thesame awesome universe, and can still enjoy myself, find insights, andexperience connectedness. And when my eight hours are done, the restof the beautiful universe is right out there for me to enjoy.

I am aware that I’ll be dead, I’m aware that my death will continue for perpetuity, and I’m aware my death will occur relatively soon.
You are correct - If I did your little experiment, there would be a little pit of despair nagging at the back of my mind.
But, let’s put your little experiment into perspective:
Instead of telling yourself 100 times each night that you’re going to die, pick a spot on your left arm and flog it with a leather strap 100 times. If you can do it without causing a significant amount of pain, bruising, and bleeding, I’ll envy you because I certainly cannot.
Would you actually conduct this experiment? If not, then why are you willing to torture your own mind by repeatedly contemplating depressing conditions beyond your ability to control?
I’m going to step back from my usual lightheartedness for a moment. All kidding, joking, and tomfoolery aside, you are describing my own behavior not even a few years ago. The “road” you appear to be on leads to chronic depression and despair. The good news is that this road has numerous busy intersections. You can stop and get directions to a better destination at any of them. Friends, family, internet acquaintances, doctors, even the police are ready and willing to help you start a new journey on a different highway.

Yes, MANY people come to the conclusion that there is no god despite years of rigorous training to the contrary. For many people, this leads to a crisis within their own minds - until they are able to suppress the training and indoctrination, they judge themselves by the yardstick of that indoctrination. Their sense of morality includes concepts of heaven and hell despite the absence of God, and they recognize that the outcome of their behavior is more consistent with eternal damnation than salvation.
Until this person can develop or adopt a moral code that operates without reliance on damnation/salvation, they will continue to use the old code, and continue to feel failure and depression about their decision.
In other words, the transition between Religious and Atheist is often a long, depressing, difficult journey, and one that can be almost entirely avoided if we have the good fortune to avoid the initial indoctrination.
As parents, it is our duty to prevent harm to our children. Allowing our children to develop critical thinking skills BEFORE exposing them to religion ensures that they will come to the decision of their own free will, and not out of some undue influence.

But epinephrine is potentially toxic, and there are case reports in the literature of babies having myocardial infarctions following repeated epinephrine inhalations, he said.
The rational use of epinephrine, therefore, involves a one-time trial in patients with moderate to severe bronchiolitis, an avoidance of repeated doses, and observation for at least 60 minutes. It can be used as rescue therapy for patients who worsen with albuterol, he noted.
Systemic corticosteroids also have been widely used, and it makes sense that their anti-inflammatory properties would be beneficial, he said. Many, but not all, small early studies were negative, but firm conclusions could not be drawn because inconsistent disease definitions and outcomes were used.
In a small but well-conducted trial, 70 patients with moderate to severe disease were randomised to 1 mg/kg oral dexamethasone or placebo, Dr. Corneli said. At 4 hours, respiratory assessment change scores improved more in the steroid group, and admission rates were much lower, at 19%, compared with 44% among placebo patients (J. Pediatr. 2002;140:27-32).
Nonetheless, experts thought that this single-centre trial’s data could not yet be considered conclusive. So in a trial under the auspices of the U.S. Pediatric Emergency Care Applied Research Network, some 600 patients were enrolled over 3 years in 20 centres, and the effects of corticosteroids were analysed. “The data remain under publication embargo, but in general we found insignificant differences in respiratory assessment change scores, later outcomes, or risk for admission with the use of steroids,” Dr. Corneli said.
As to why corticosteroids may not be useful in alleviating the airway inflammation associated with bronchiolitis, evidence is emerging suggesting that replication of respiratory syncytial virus actually may increase with corticosteroid therapy. There also is some evidence that inflammation may be beneficial in slowing the spread of the virus out of the nasopharynx down into the bronchi and bronchioles, Dr. Corneli said.
Finally, there are simple measures that can help in the ED management of bronchiolitis, such as suction, stimulation, and positioning of the child, along with the use of mist and fluids.
Parental education also is important. The message for parents is “don’t panic,” because 50%-66% of kids have RSV infection by 1 year, and 80%-100% are infected by the second or third year of life, Dr. Corneli said. Only 12%-40% develop bronchiolitis with this infection, only 5% become significantly ill, and only 1% require hospitalisation, he said.
