Despite these limitations, these data suggest that there are important, yet complex interactions between sleep and FAP. There is evidence of an increased prevalence of sleep disturbances among patients with FAP. Sleep disturbances in patients with FAP appear to be exacerbated by pain, however, the sleep–pain relationship is often considered to be bi-directional and it remains unclear as to whether pain is the antecedent to sleep disruption or if the inverse relationship is true. Increased arousal could result in disruptions in sleep quality and continuity or pain, with each of these then influencing or exacerbating the other. For example, symptoms of insomnia may cause chronic sleep disruption that leads to decrements in focused attention and adaptive coping, both critical for an individual's ability to deal with pain and other physical symptoms of FAP. Thus it is important to consider that sleep disturbances may be a precursor to chronic pain disorders such as FAP that have no identified organic cause.
Future research should focus on testing multivariate etiologic models that specify how the various risk factors might contribute to functional outcomes. For example, it would be useful to test whether increases in anxiety or depressive symptoms mediate the relationship between FAP symptoms and the onset of BSDs. Finally, it will be vital to conduct randomized trials that test the efficacy of treatment strategies targeting adaptive coping for FAP, and therapy for mood and sleep disorders. An improved understanding of the etiologic processes that give rise to sleep disorder symptoms and the groups that are at greatest risk for BSD should ultimately facilitate the design of effective prevention programs for this common pediatric health problem.

Finding the true dying point. Providers felt that in the acute care environment it is difficult to know when to make the transition from curative care to palliative care. Participants felt that the decision to transition is made more easily when there is a point in time in the child's illness trajectory when it is apparent that the focus of care should be shifted from aggressive, life sustaining treatment toward making the child comfortable and taking care of the family. But this "dying point" is often not unanimously agreed upon by family and providers. One person stated:

October 11, 2007 — Intake of milk in pregnancy was associated with higher birth weight for gestational age, lower risk for small for gestational age (SGA), and higher risk for large for gestational age (LGA), according to the results of a prospective cohort study published in the October issue of the American Journal of Clinical Nutrition.
"Cow milk contains many potentially growth-promoting factors," write Sjurdur F. Olsen, from the Institute of Public Health, University of Aarhus in Denmark, and colleagues from the NUTRIX Consortium. "There is a lack of large prospective studies with a sizeable group of women with zero or low milk consumption. We therefore examined in detail, in a large prospective Danish pregnancy cohort, the relations of milk intake and milk constituents with measures reflecting intrauterine growth."

October 9, 2007 (San Diego) — The larger doses of vancomycin administered to achieve compliance with revised guidelines for trough concentrations of the drug are associated with increased risk for nephrotoxicity, according to a presentation here at the Infectious Diseases Society of America 45th Annual Meeting.
The study was prompted by changes in vancomycin treatment guidelines that recommend achieving trough concentrations of between 15 and 20 mg/L. This is because of relatively poor tissue penetration, particularly in the lungs, and associated "'mycin creep,' in which the vancomycin [minimum inhibitory concentration] that is fully susceptible to [Staphylococcus] aureus increases over time," said presenter Thomas P. Lodise, PharmD, from the Albany College of Pharmacy, New York. Anecdotal reports have suggested that trough levels of 15 mg/L or more were associated with higher rates of nephrotoxicity.
